In the United States, just 5% of patients account for 50% of healthcare costs. Lauran Hardin explains why a deep understanding of the causes of chronic disease and the needs of patients requiring complex care can radically transform outcomes not only for those most at risk, but also for communities as a whole, leading to major reductions in matters of public safety, health care and others. costs. Hardin believes that understanding chronic care opens new opportunities for rethinking health care for all. Lauran Hardin is the Senior Advisor to the Camden Coalition’s National Center for Complex Health and Social Needs. In this role, she draws on her extensive experience in consulting, co-design and coaching in care redesign and complex care transformation. Current projects include co-designing a complex care ecosystem for uninsured patients in Memphis, Tennessee, scaling a community-based, cross-sector care coordination model for vulnerable populations across the Adventist Health System and the development of a complex care clinic model within the Sutter Health System.

Hardin previously served as director of a complex care center serving hospitals, multiple providers and more than 1,500 complex/high-frequency patients in the Mercy Health System. The Center's complex care model has led to improved patient navigation and outcomes, including reduced emergency room visits, hospitalizations and costs for vulnerable populations. Aspects of the care model have been implemented in more than twenty Trinity Health ministries in six states, including rural and urban areas.
Hardin received the AACN National CNL Vanguard Award in January 2015. Her innovative care model earned her “Edge Runner” recognition from the American Academy of Nursing in June 2015. She was named an AARP Culture of Health Scholar at the National Center. for Complex Health and Social Care Needs in January 2017 and was inducted as a Distinguished Fellow of the National Academies of Practice in Nursing in 2018. Hardin received her master's degree in nursing from the University of Detroit Mercy, with certifications as a CNL, Pain Management and Hospice. She trained as a facilitator at the Elisabeth Kubler-Ross Center, spent several years working in a hospice center, and co-developed the first pain and palliative care service in the West Michigan area .
